What's Covered

Starting TRT is a clinical process, not a single event. How your body responds to treatment — at the dose, formulation, and frequency initially prescribed — needs to be assessed and your protocol adjusted accordingly. This consultation is where that happens.

01Symptom response review

Your doctor will take a systematic account of how your symptoms have changed since starting or adjusting your TRT. Energy, mood, libido, sleep, cognitive function, and physical performance are all reviewed. Response to TRT is individual — some men respond well to an initial protocol; others require dose or formulation adjustment to achieve optimal results.

02Blood level review

Testosterone levels — total and free where relevant — are checked against your baseline and previous results to confirm adequate levels are being achieved. Oestradiol is reviewed, as testosterone converts to oestrogen via aromatisation and elevated levels can cause symptoms that mimic low testosterone. SHBG, full blood count, and haematocrit are assessed for safety.

03Safety monitoring — haematocrit and PSA

TRT can raise haematocrit (the proportion of red blood cells), which above certain thresholds increases cardiovascular risk. PSA is monitored for relevant changes in the context of prostate health. These are non-negotiable components of responsible TRT management and are reviewed at every follow-up.

04Protocol refinement

Based on your symptom response and blood results, your doctor will adjust your TRT protocol where appropriate — modifying dose, changing injection frequency, switching formulation, or adding adjunctive treatment (such as hCG for testicular volume).

05Questions and concerns

This appointment is an opportunity to raise anything that has come up since your last consultation — side effects, questions about your prescription, or areas you want to explore further.

When to book: typically 6–8 weeks after starting TRT, every 3–6 months during the first year, and at minimum annually thereafter.

Difference

Why Monitoring Is Not Optional

TRT without monitoring is not responsible medicine. Blood monitoring is the only way to ensure safety and optimise response.

Haematocrit & PSA tracking

Testosterone stimulates red blood cell production. Regular monitoring detects polycythaemia (elevated haematocrit) before it becomes a problem. PSA is tracked so any significant change in prostate health is identified early.

Oestradiol management

Excess aromatisation of testosterone to oestradiol can cause side effects like water retention or mood changes. Monitoring allows for early identification and management.

Dose optimisation

Starting doses vary in effect between individuals. Monitoring is the only way to know if your levels are subtherapeutic or supraphysiological and adjust accordingly.

Questions

Common Questions

How soon after starting TRT should I book a follow-up?

We recommend booking your first follow-up approximately 6–8 weeks after initiating treatment. For injectable testosterone, this should be timed towards the middle of your injection cycle — not immediately before or after an injection — to give a representative steady-state level.

Which blood tests do I need before this appointment?

A standard TRT monitoring panel includes total and free testosterone, oestradiol, SHBG, full blood count with haematocrit, PSA, and liver function. Your doctor will advise on the specific panel required. These can be arranged at EGA.

My symptoms have improved. Do I still need a follow-up?

Yes. Haematocrit and PSA monitoring are required regardless of symptom experience. Levels can change independently of how you feel, and safety monitoring is a clinical obligation of responsible TRT prescribing.

My symptoms have not improved. What does that mean?

Inadequate response may reflect insufficient dose, suboptimal formulation, elevated oestradiol, or a contributing factor unrelated to testosterone levels. A follow-up consultation is the appropriate setting to assess this.

Can I transfer my TRT care to EGA from another clinic or my NHS GP?

Yes. If you are currently on TRT prescribed elsewhere and wish to transfer ongoing monitoring and prescribing to EGA, a follow-up consultation is the appropriate entry point. Please bring your current prescription details and recent results.

How often will I need monitoring long-term?

Once your protocol is stable and your markers settled, six-monthly or annual reviews are typically sufficient. More frequent monitoring is appropriate during the first year or after any dose change.

Is this consultation available remotely?

Please contact us to discuss. Where clinically appropriate, some follow-up consultations may be available by video, provided blood testing has been completed.

What happens if my haematocrit is too high?

If your haematocrit exceeds safe thresholds, your doctor will discuss management options, which may include dose adjustment, formulation change, or temporary cessation until levels normalise.
