What's Covered

B12 is unique among vitamins: it is the only one that requires a specific protein produced by the stomach — intrinsic factor — to be absorbed in the gut. Intrinsic factor production declines with age. PPIs, metformin, gut inflammation, and coeliac disease all reduce it further. A plant-based diet provides virtually no dietary B12. The result: deficiency is common — affecting an estimated 6% of adults under 60 and nearly 20% over 60 in the UK (BMJ, 2014) — and frequently goes undiagnosed for years.

01Energy metabolism

B12 is a cofactor for the conversion of methylmalonyl-CoA to succinyl-CoA — a step in the Krebs cycle essential for cellular energy production from fats and proteins. Without adequate B12, this step stalls and cellular energy production is impaired upstream of the mitochondria.

02Nervous system and myelin

Required for synthesis and maintenance of myelin — the protective sheath around nerve fibres. B12 deficiency causes progressive demyelination, initially presenting as tingling, numbness, and cognitive symptoms, which can progress to irreversible neurological damage if uncorrected.

03Methylation cycle

Cofactor for methionine synthase, which regenerates methionine from homocysteine. Without adequate B12, homocysteine accumulates — an independent risk factor for cardiovascular disease, stroke, and cognitive decline. B12 also supports production of SAM-e, the primary methyl donor required for neurotransmitter synthesis and DNA methylation.

04Red blood cell formation

Essential for DNA synthesis in red blood cell precursors. Deficiency causes megaloblastic anaemia — large, poorly functional red blood cells — producing fatigue, breathlessness, and pallor.

Common symptoms of suboptimal B12 include fatigue, brain fog, poor concentration, tingling or numbness in hands and feet, low mood, anxiety, mouth ulcers, a sore inflamed tongue, pale skin, breathlessness, and palpitations.

Audience

Who It's For

Particularly relevant where gut absorption is impaired or dietary intake is limited.

  • Confirmed or suspected B12 deficiency on blood testing
  • Fatigue, brain fog, or mood symptoms that may reflect suboptimal B12 status
  • Over 50, where intrinsic factor production and absorption naturally decline
  • Taking PPIs, metformin, or other medications that impair B12 absorption
  • Vegans and vegetarians with limited dietary B12 intake
  • Post gastric bypass or other gastrointestinal surgery
  • Want a quick, reliable B12 repletion without the time commitment of an IV drip
  • Combining B12 with an EGA IV drip for additional targeted support

Deliverable

Results and What to Expect

Effect is most pronounced in people with genuine B12 insufficiency.

  • Many clients report improved energy and mental clarity within 24 to 48 hours
  • Mild, brief sting at the injection site resolves within seconds
  • Monthly injections are typical for ongoing maintenance
  • Initial repletion of significant deficiency may use a more frequent loading course
  • Often combined with an IV drip session as an add-on

Important information: IV and injected nutrient treatments are not a substitute for a balanced diet or for medical treatment. Responses vary between individuals, and evidence of benefit in people without a diagnosed deficiency is still developing. Every treatment follows a clinical assessment, and your clinician will advise whether it is appropriate for you.

Difference

Why B12 Deficiency Is So Common

An estimated 6% of UK adults under 60 and nearly 20% over 60 are B12 deficient (BMJ, 2014). It is frequently missed because symptoms develop gradually and are attributed to other causes.

Intrinsic factor decline

Production of the protein required for B12 absorption declines with age, making deficiency increasingly common after 50.

Common medication impact

PPIs (omeprazole, lansoprazole) and metformin — among the most widely prescribed medications in the UK — directly impair B12 absorption.

Gut conditions

Inflammation, coeliac disease, and gastric bypass surgery all reduce B12 absorption significantly.

Dietary patterns

A plant-based diet provides virtually no dietary B12. Even omnivores may have insufficient intake from food alone.

05

Questions

Common Questions

Is the injection painful?

Mildly. The needle is small and the injection is brief. Many clients describe a slight sting that resolves within seconds.

How quickly will I notice an effect?

Many clients report improved energy and mental clarity within 24 to 48 hours, particularly those who were significantly deficient. The effect is most pronounced in people with genuine B12 insufficiency.

How often should I have the injection?

For maintenance with ongoing absorption issues, monthly injections are typical. For repletion of significant deficiency, a loading course of more frequent injections may be appropriate. Your clinician will advise based on your blood results.

Do I need a blood test first?

Not for a single session. We recommend knowing your B12 level before beginning a regular course, both to confirm deficiency and to track response. EGA can arrange blood testing if needed.

Can I combine this with an IV drip on the same visit?

Yes. A B12 injection is often combined with an IV drip session as an add-on for additional targeted support.
