What's Covered

Starting BHRT is the beginning of a process, not a single event. How your body responds to treatment — at the dose and formulation initially prescribed — needs to be assessed, and your protocol adjusted accordingly. This consultation is where that happens.

01Symptom response review

Your doctor will take a systematic account of how your symptoms have changed since starting or adjusting your BHRT. Which symptoms have improved? Which remain? Have any new symptoms emerged? Response to BHRT is individual — some women respond well to an initial protocol; others require dose or formulation adjustment to achieve adequate symptom control.

02Blood level review

Hormone levels are checked against the results taken at the time of your initial assessment or last follow-up. Oestradiol, progesterone, and testosterone levels — along with relevant safety markers including liver function, lipid profile, and full blood count — are reviewed to confirm that your levels are in the optimal therapeutic range and that the treatment is being absorbed and metabolised as expected.

03Protocol refinement

Based on your symptom response and blood results, your doctor will adjust your BHRT protocol where appropriate — modifying dose, changing formulation, adjusting timing, or adding a further hormonal component if clinically indicated.

04Ongoing safety monitoring

Regular monitoring is an integral part of responsible BHRT prescribing. Your doctor will review any relevant safety markers, discuss any changes in your personal or family medical history since your last appointment, and confirm that your current protocol remains appropriate.

05Questions and concerns

This consultation is also an opportunity to raise anything that has come up since your last appointment — side effects, questions about your prescription, symptoms you have noticed, or areas you want to explore further.

When to book: typically 6–8 weeks after starting BHRT, then every 3–6 months during the first year, and annually thereafter.

Difference

Why Monitoring Matters

BHRT is not a static prescription. Hormone requirements change over time with progression through perimenopause, weight changes, or the natural evolution of your hormonal picture.

Proactive, not reactive

Regular monitoring allows your doctor to stay ahead of changes — adjusting your protocol proactively rather than reactively, ensuring your treatment reflects your current clinical picture.

Data-informed titration

A protocol that is appropriate at initiation may need adjustment six months later. Blood levels that are adequate in one season may shift in another.

Clinical safety

Initiating treatment without structured follow-up is not an approach EGA takes. We ensure your hormones remain within safe, evidence-based parameters.

Questions

Common Questions

How soon after starting BHRT should I book a follow-up?

We recommend booking your first follow-up approximately 6–8 weeks after initiating treatment. This gives your body time to reach stable hormone levels and gives your doctor meaningful data on both your blood levels and your symptom response.

Do I need blood tests before this appointment?

Yes. A hormone and safety blood panel is required before your follow-up consultation so that your doctor can review your levels at the appointment rather than waiting for results afterwards. If you are a current EGA patient, your doctor will advise on the specific markers to test. This can be arranged at EGA or through your GP.

My symptoms have improved significantly. Do I still need a follow-up?

Yes. Even when symptoms are well controlled, monitoring blood levels and safety markers is an integral part of responsible BHRT management. Hormone levels can change independently of symptom experience, and ongoing monitoring ensures your protocol remains appropriate and safe over time.

My symptoms have not improved. What does that mean?

Inadequate symptom response can have several causes — insufficient dose, suboptimal absorption, an additional hormonal component being needed (such as testosterone), or a non-hormonal factor contributing to your symptoms. A follow-up consultation is exactly the right setting to assess this and make the necessary adjustments.

Can I use this appointment to transfer care from another clinic or my NHS GP?

Yes. If you are currently on HRT or BHRT prescribed elsewhere and wish to transfer your ongoing care to EGA, a follow-up consultation is the appropriate entry point. Please bring your current prescription details and any recent blood results.

How often will I need follow-up consultations long-term?

Once your protocol is stable and your symptom picture settled, annual reviews are typically sufficient. During the first year, or any time your protocol changes, more frequent reviews are appropriate. Your doctor will advise on the right schedule for your individual situation.

Is this consultation available remotely?

Please contact us to discuss. Where clinically appropriate, some follow-up consultations may be available by video.

What if I miss my review window?

It is important to maintain regular reviews to ensure ongoing prescribing safety. If you have missed your scheduled review, please book as soon as possible to avoid disruption to your treatment protocol.
