What's Covered

Bioidentical hormone replacement therapy (BHRT) uses hormones molecularly identical to those your body produces naturally — oestradiol, progesterone, and testosterone. This package covers the full arc of responsible BHRT initiation: assessment, prescription, response, and refinement.

011. BHRT Initial Consultation (50 minutes)

A specialist, doctor-led assessment covering your full hormonal symptom picture, medical and gynaecological history, family risk profile, previous treatments, and your goals. Your doctor will explain your BHRT options in detail and make a personalised treatment recommendation. Where appropriate, a prescription may be initiated at this appointment.

022. BHRT Follow-up Consultation (30 minutes)

A structured clinical review, scheduled 6–8 weeks after starting treatment. Your doctor reviews your blood hormone levels, assesses your symptom response, and refines your protocol based on what the data and your experience show. Dose adjustments, formulation changes, or the addition of further hormonal components are addressed at this appointment.

03Full hormonal symptom review

Identification of the clinical picture formed by hot flushes, night sweats, disrupted sleep, mood changes, brain fog, fatigue, low libido, and joint pain. Many women live with these symptoms for years before connecting them to hormonal change; this package is designed to identify and address that cause.

04Comprehensive medical and history review

Your full personal and family medical history is reviewed — including history of breast cancer, cardiovascular disease, or osteoporosis — as these are clinically relevant to BHRT prescribing decisions. Menstrual and contraceptive history are also discussed.

05Discussion of BHRT options

Explanation of available BHRT formulations — oestradiol, progesterone, and testosterone — and the different routes of administration (transdermal gels, patches, oral micronised progesterone, and others) in relation to your specific situation.

Total saving of £•• versus booking separately (Initial Consultation £••• + Follow-up Consultation £••• = £••• if booked individually).

Difference

The EGA Approach to BHRT

NICE guidelines (NG23, 2024) recommend HRT as first-line treatment for menopausal symptoms. At EGA, we assess your status in the context of your long-term health — bone, cardiovascular, cognitive, and metabolic.

Assessment and titration

The initial consultation identifies where you are; the follow-up confirms whether the treatment is working as intended. These are not interchangeable and both are essential for responsible care.

Evidence-informed prescribing

BHRT at EGA is always preceded by a full clinical assessment, guided by blood test data, and prescribed within evidence-based parameters in line with British Menopause Society guidance.

Structured monitoring

No BHRT protocol is initiated at EGA without a full assessment and a plan for structured follow-up. This package ensures your first review is booked before you even begin.

Questions

Common Questions

What is the difference between BHRT and standard HRT?

Standard NHS HRT uses hormones that may be synthetic or derived from non-human sources. BHRT uses hormones molecularly identical to those your body produces — oestradiol, progesterone, and testosterone. At EGA, we prescribe regulated, licensed bioidentical preparations (body-identical) rather than compounded preparations, in line with British Menopause Society guidance.

How far apart are the two appointments?

The initial consultation and the follow-up are typically 6–8 weeks apart, with BHRT initiated and blood testing completed in between. This timing allows hormone levels to stabilise before the follow-up assessment.

Do I need blood tests before my initial consultation?

Not necessarily. Your doctor will assess what testing is appropriate during the consultation. If you have recent hormone results, bring them — they may reduce or eliminate the need for further testing before your prescription is issued. If new testing is required, this can be arranged at EGA.

I am in my 40s and my GP says my hormone levels are normal. Could I still be perimenopausal?

Yes. Perimenopause is primarily a clinical diagnosis — made on the basis of symptoms and history, not blood test results alone. FSH and oestradiol levels fluctuate significantly, and a single 'normal' result does not exclude hormonal change. NICE guidelines note that blood tests should not be used in isolation to diagnose perimenopause in women over 45.

Is BHRT safe?

BHRT using licensed body-identical hormones — particularly transdermal oestradiol and oral micronised progesterone — has an established safety profile. Transdermal oestradiol does not carry the increased VTE (blood clot) risk associated with oral oestrogen. Your individual risk profile will be discussed in full during your consultation.

Will I leave the first appointment with a prescription?

Your doctor may initiate a BHRT prescription at the end of the initial consultation if assessment supports this and no further investigation is required first. In some cases, blood testing will be recommended before a prescription is issued.

I have a family history of breast cancer. Can I still consider BHRT?

A family history of breast cancer is a relevant consideration, not an automatic contraindication. Your doctor will assess your individual risk profile — including the nature of the family history and the specific formulations being considered — and discuss this with you openly.

Are blood tests included in the pack price?

Blood tests are not included in the consultation pack price and are charged separately. Your doctor will advise on the specific tests required, which can be arranged at EGA.
