What's Covered

Bioidentical hormone replacement therapy (BHRT) uses hormones that are molecularly identical to those your body produces naturally — oestradiol, progesterone, and testosterone. At EGA, every protocol is built around your individual hormone levels, symptom picture, medical history, and goals — assessed in full before any treatment decision is made.

01Full hormonal symptom review

Your doctor will take a detailed account of your current symptoms — hot flushes, night sweats, disrupted sleep, mood changes, anxiety, brain fog, fatigue, low libido, vaginal dryness, joint pain, weight changes, and others. Many women present with symptoms across multiple systems that, together, form a clear hormonal picture.

02Comprehensive medical and gynaecological history

Your full personal and family medical history is reviewed — including any history of breast cancer, cardiovascular disease, blood clots, or osteoporosis — as these are clinically relevant to BHRT prescribing decisions. Your menstrual history, contraceptive history, and any previous hormonal treatment are also discussed.

03Review of previous blood results and treatments

If you have had prior hormone testing, HRT prescriptions, or other treatments — whether through your NHS GP, another private clinic, or EGA — your doctor will review these in context. If results are incomplete or out of date, appropriate testing will be recommended.

04Discussion of BHRT options

Your doctor will explain the available BHRT formulations — oestradiol, progesterone, and testosterone — and the different routes of administration (transdermal gels, patches, oral micronised progesterone, topical testosterone, and others). The relative benefits and considerations of each will be discussed in relation to your specific situation.

05Personalised treatment recommendation

At the end of the consultation, your doctor will make a clear recommendation — whether that is to proceed with a BHRT protocol, undertake further testing first, or explore other interventions. EGA takes an evidence-informed approach to BHRT prescribing in line with current British Menopause Society guidance.

Any prescription decision is made on clinical grounds. In some cases, blood testing will be recommended before a prescription is issued. Your doctor will be clear about the recommended sequence at the end of your appointment.

Difference

The EGA Approach to BHRT

NICE guidelines (NG23, updated 2024) recommend HRT as the first-line treatment for menopausal symptoms. At EGA, we assess your hormone status in the context of your long-term health — bone density, cardiovascular risk, cognitive function, and metabolic health.

Comprehensive assessment before any prescription

No BHRT protocol is initiated at EGA without a full assessment. BHRT is guided by blood test data, not symptoms alone, and prescribed within evidence-based parameters.

Individualised protocol

50 minutes allows detailed symptom review, history, and discussion of treatment options. BHRT regimens are tailored to your hormone levels, symptom picture, medical history, and goals.

Structured monitoring

All BHRT at EGA is subject to structured monitoring and follow-up. This consultation begins a clinical relationship that continues through treatment and review.

Questions

Common Questions

What is the difference between BHRT and standard HRT?

Standard NHS HRT uses hormones that may be synthetic or derived from non-human sources, prescribed in fixed formulations. BHRT uses hormones molecularly identical to those your body produces — oestradiol, progesterone, and testosterone. At EGA, we prescribe regulated, licensed bioidentical preparations (such as body-identical oestradiol and micronised progesterone) rather than compounded preparations, in line with British Menopause Society guidance.

Do I need blood tests before this consultation?

Not necessarily. Your doctor will assess what testing is appropriate during the consultation. If you have recent hormone blood results — from your GP or elsewhere — please bring them. If testing is needed, this can be arranged at EGA following the consultation or, where possible, at the same visit.

I am in my 40s and my GP says my hormone levels are normal. Could I still be perimenopausal?

Yes. Perimenopause is primarily a clinical diagnosis — made on the basis of symptoms and history, not blood test results alone. FSH and oestradiol levels fluctuate significantly throughout perimenopause, and a single 'normal' result does not exclude hormonal change as the cause of your symptoms. NICE guidelines specifically note that blood tests should not be used in isolation to diagnose perimenopause in women over 45.

Is BHRT well tolerated?

BHRT using licensed body-identical hormones — particularly transdermal oestradiol and oral micronised progesterone — has an established safety profile. Transdermal oestradiol does not carry the increased VTE (blood clot) risk associated with oral oestrogen preparations. The risk profile of any hormonal treatment is individual and will be discussed in full during your consultation.

Will I leave this consultation with a prescription?

Your doctor may initiate a BHRT prescription at the end of the consultation if assessment supports this and no further investigation is required first. In some cases, blood testing will be recommended before a prescription is issued. Your doctor will be clear about the recommended sequence at the end of your appointment.

I have a family history of breast cancer. Can I still consider BHRT?

A family history of breast cancer is a relevant consideration in BHRT prescribing, not an automatic contraindication. Your doctor will assess your individual risk profile — including the nature of the family history, your own risk factors, and the specific hormones and formulations being considered — and discuss this with you openly.

Do I need a GP referral?

No. You can book directly online or by phone.

Is follow-up included?

No. A separate BHRT Follow-up Consultation (£•••) is used for review and refinement, or the Comprehensive Pack (£•••) bundles both at a reduced price.
